Dramatically, though, it worked. It was a kick in the stomach. I can't think of a character death in a genre film that's ever had that much of an effect on the audience as that one.

If one of those deaths didn't actually occur in the final cut of the movie. :-)
I mean, the first one. You know the one. The only actual confirmation we have of it happening is that memorial at the end, and the would be an easy scene to reshoot - I mean, why would they leave a memorial to him on that planet anyway?
I'm just sayin'... ;-) And that kind of "mess with the minds of the True Fans, and then change it, so they come storming back to see it again" is terribly, terribly in-character for Joss.
Well, I think Book's passing (hey, I said it was Spoiler Zone!) acted as an impetus. It gave Mal something to Believe in, and forced that lesson home about how he needed it.
Not to mention, it fit in with the destruction of every place they considered home. They'd have had to bring Book with them, since they couldn't really leave him to recuperate in Haven's ruins.
His death is too much a part of the story to be eliminated. Sure, Book had a lot of story left to tell. Possibly that'll get told elsewhere (prequel?).
